He was predeceased by: his parents, Donald David, Sr. and Betty P. Houston (Evans); and his sister Carolyn Frederick. He is survived by: his niece Michelle Frederick of Dieterich, IL; his great-niece Katelyn Frederick of Dieterich, IL; and his uncle Junior Evans of Mt. Vernon, IL.

Memorials may be given to the Shelbyville Booster Foundation.
