He is survived by: his wife Juanita Chism; his children, Brenda Coker (Robert) of Springfield, Danny Formea (Cheryl) of Springfield, Merri Cook (Joseph) of Springfield, David Formea (Vickie) of Springfield and Tommy Formea (Beth) of Cape Coral, FL; his grandchildren, John Hauer (Dawn) of Springfield, Amy Kelly (Kevin) of Cape Coral, FL, Ben Formea of Chatham, Cory Formea (Jessica) of Springfield, Chad Formea (Tracy) of Chatham, Brett Wilson of Springfield, Brian Wilson (Chelsea) of Springfield, Jennifer Sagle of Buffalo Heart, IL, Teri Belisle of St. Louis, Anthony Formea of Springfield, Kimberly Griffitts (Ben Carter) of New Berlin, Sarah Rickaby of Oak Creek, WI and Jamie Formea (Jessica) of Heartland, WI; and his siblings, John Formea of Albuquerque, NM, Joe Formea (Betty) of Carlinville, IL, Kathleen Toigo of Godfrey, IL and Marjorie Davis (John) of Hardin, IL. He is also survived by twenty two great-grandchildren; two great-great-grandsons.
