He was predeceased by: his father Thomas Hawk; his stepson Richard "Opie" Haworth; and his sister Susan Brooks. He is survived by: his wife Margaret Hawk; his children, Kimberly Mayhak (Michael), Robert Bennett (Jennifer), Michael (Holly Clarke) and Justin Hawk (Courtney); his stepchildren, William Haworth (Pearl), Denise Pflueger (Mark), Carol Haworth and Margie Haworth; and his sibling Thomas Hawk (Brenda).
