He was predeceased by: his father Frank Slovnik Jr.; and his grandparents, Alex, Jean Skokie and Frances Slovnik. He is survived by: his mother Jean Slovnik; his siblings, Kristen Tuohy (Kevin), Frank Slovnik (Nikki) and Amy Gramer (Jim); and his siblings' children, Dylan Tuohy (Kiernan), Raquel Tuohy, Jacob, Abby Gramer, Ashley, Cole and Mason Slovnik.
