He was predeceased by: his parents, Theodore W. Cook and Minnie E Cook; and his sisters, Ruth Hoppes-McCord and Mary Behrens. He is survived by: his wife Ruth Ellen Cook (McIntyre) of Anderson; his sons, Jon T. Cook (Pam) of Anderson and Jay M. Cook (Kathy) of Columbus; and his grandsons, Joshua Cook of Lafayette, Justin Cook (Brittney Shoemaker) of Anderson, Jesse Goodpaster of Columbus and Jacob Cook of Columbus. He is also survived by eight great-grandchildren; several nieces and nephews.
