He was predeceased by: his stepfather Ralph "Buz" Holtz; and his stepbrothers, Brad Holtz and Scott Holtz. He is survived by: his parents, Tina M. Chyzy-Holtz (Ralph "Buz" Holtz) and Robert "Jay" Hopkins (Deborah Johnson); his wife Jennifer; his children, Jessica, Jason Jr. and Janis; his brother Joshua Hopkins; and also, Deb Johnson, Renee, Britley and Brianna.
