He was predeceased by: his parents, Claude Cook and Lucile Cook; and his sister Jane. He is survived by: his wife Christina (Weldy); his sons, Ken Cook (Cindy) of Agoura Hills, CA and Brad Cook of Butlerville, IN; and his grandchildren, Forrest Cook (Courtney), Elizabeth Cook and Kailey Cook. He is also survived by two great-grandchildren.
In lieu of flowers, the family requests contributions be made to Brown County Humane Society in Nashville, IN, Wounded Warrior Project, the American Cancer Society, or Best Friends Animal Society in Kanab, UT.
