He was predeceased by: his wife Anna Marie Kubiak; his brother Thomas Kubiak; and his parents, Walter Stanley and Ann J. Kubiak. He is survived by: his children, Jennifer Kubiak (Louis Gain) and Michael Kubiak (Danielle); his grandchildren, Madison Kubiak, Kaitlyn Kubiak, Allison Kownacki, John Kownacki, Rylee Kubiak and Michael Gain; and his pet Lucy. He is also survived by many nieces, nephews and dear friends.
In lieu of flowers, memorials may be made to the Wounded Warrior Project.
