He was predeceased by: his parents, Frank Mikolasek and Eleanor Mikolasek (Nelson); and his brother Jim Mikolasek. He is survived by: his wife Audrey Mikolasek (Hartline); his children, Leah Hoffman (Mark), Laura Ann Mikolasek and Steven Mikolasek of Evansville; and his sisters, Marilyn Ebbinger and Donna Karner.
