He was predeceased by : his mother Christine Cook (Kemp). He is survived by : his father Donnie Cook (Cathy); his daughter Kennedy Cook; and his siblings, Donnie Cook II (Carolyn), Hannah Cook and Emily Cook. He is also survived by aunts, uncles, many nieces and nephews, and cousins.
