He is survived by: his sons, Phillip J. Collins, Bradley T. Collins (Amanda) and Randall W. Collins; his brother Michael (Charlotte); his grandchildren, Jennifer Collins, Natasha Ulrey (Greg), Shantel Hollingsworth, Tiffany Kittinger (Clay), Brady Collins, Madison Collins, Brooklyn Collins and Brandon A. Worthington; and his daughter-in-law Mary Collins-Burnworth (Jody).
