She was predeceased by: her parents, Ralph Moorman and Mary Moorman (Barrett); her siblings, Richard Moorman, Bob Moorman (Ruby), Rosemary Boles (Bill) and Dorothy Crowder (Harris); and her son-in-law Paul Gallo.
She is survived by: her spouse Zell Hazelbaker; her children, Carol Ann Gallo and Duane Hazelbaker (Janna); her grandchildren, Matt Gallo (Amy), Darci Hazelbaker (Dale Rush) and Danen Hazelbaker (Lana Sloutsky); her grandmother Wanda; Fairmount and Jonesboro. She is also survived by many nieces and nephews.
In lieu of flowers, memorial contributions may be made to Epworth Methodist Church in Matthews, Indiana.
