He is survived by: his mother Anna Vonita Seipel (Watts) of Greenwood; his children, Leslie Blazek, Erin Blazek of Indianapolis, Samuel Blazek (Mandy) of Connersville, Diann Blazek (Marlon Robinson) of Imperial Beach and Thomas Blazek of Virginia Beach, Virginia; his siblings, Vickie Miller (Jeff) of Indianapolis, Sidney Blazek (Jeanne) of Greenwood, Robert Blazek (Sue) of Indianapolis and Jerold Blazek of Franklin; and his grandchildren, Jade Luna, Andrew Damron, Charles Lee III, Maizy Blazek and Ramzy Blazek. He is also survived by his children and grandchildren.
