He was predeceased by: his parents, James Owen and Dorothy Wilson Sullivan; his son Larry Wayne Sullivan; and his brothers, Kenneth, Robert and Orville Sullivan.

He is survived by: his wife Charlotte Davis Sullivan of Louisville, KY; his son Shayne Sullivan (Kimberly) of Floyds Knobs, IN; his grandchildren, Shelby, Scarlett, Shayne W. Sullivan, Jr. (Liam) and Olivia Carroll; his siblings, Ray Sullivan, Belinda Sullivan and Dorothy Sutt; and his sister-in-law Shirley.
