She was predeceased by: her parents, Finley McIntosh and Elvira McIntosh (Spicer). She is survived by: her former husband John Smith; her husband Lou; and her children, Vickie "Diane" Cooper (Richard), John Smith Jr. (Carmen) of Lafayette and Michael L. Smith of Lafayette. She is also survived by 4 grandsons; 3 granddaughters; and 2 great grandchildren.

Memorial contributions may be given in Faye's name to the American Cancer Society.
