She was predeceased by: her husband Mickey Kostelnik Sr.; her parents, Isaac Martinez and Rosa Martinez; and her siblings, Angie, Amparo, Joseph and Frank.
She is survived by: her sons, Mickey Kostelnik Jr. (Mary Ellen), Emery Kostelnik (Terry) and Brian Kostelnik (Pat). She is also survived by grandchildren, and 6 cherished great-grandchildren.
Interment Calumet Park Cemetery.In lieu of flowers, donations in Teresa's name can be made to St. Jude Children's Research Hospital.
