It is with great sadness that we announce the death of Clem Phillip Cook (New Albany, Indiana), who passed away on March 6, 2020, at the age of 80, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
He was predeceased by: his parents, Clem Phillip Cook Sr. and Johnnymae Cook; his wife Sandra Amburgey; and his siblings, Kenny Cook and Denise Cook.
He is survived by: his children, Mark Cook of Cincinnati, Phillip Cook of Akron, Tim Cook (Tracy) of Seattle Washington, Daniel Todd of Virginia, Tameeka Cook (Ron Glass) of Charlestown, Shartanya Bennett of New Albany, Barbara Crawford of Cincinnati and Joni Shelley of North Vernon. He is also survived by several grandchildren; several great-grandchildren; and several great-great-grandchildren.
