He was predeceased by: his parents, Willard Collins and Myrtle Collins; and his brothers, Melvin and Neal Collins. He is survived by: his wife Connie Collins; his children, Jack D. Collins Jr. (Rhonda), Edward W. Collins and Paula Lee Schuk (Ronnie); and his siblings, Thomas Collins (Judy), Robert Collins and Patricia Smith. He is also survived by several grandchildren and great grandchildren.
