He is survived by: his wife Lynne A. Mayfield (Kreutzer); his son Brad Alan Mayfield; his siblings, Judy Fields and Dick Mayfield (Marguerite); and his nieces and nephews, Monty Fields (Emily), Brian Mayfield (Jess), Linda McGowan and Stephanie Mauk (Tony).

In Lieu of flowers, the family requests donations go to New Albany Animal Shelter.
