He is survived by: his sons, Chad Cook of Batesville, AR, Jeremy Cook (Vivian) of Evansville, IN and Andy Cook (Heather); his sister Cheryl Cook of Evansville, IN; and his grandchildren, Nolan, Emery, Colton and Anderson.
