He is survived by: his daughters, Lisa Hazelwood of Paoli, Penny Beatty of Paoli and Charity Welch of Campbellsburg.
Survivors include: Daughters- Lisa Hazelwood & Penny Beatty both of Paoli & Charity Welch of Campbellsburg 1 Grandchild.
