He was predeceased by: his father Ervin D. Mayhall; and his father-in-law Eugene Smith. He is survived by: his wife Jennifer Mayhall (Smith) of Princeton; his sons, Zachary W. Mayhall (Lindsey) of Evansville and Kyle E. Mayhall of Nashville, TN; his grandchildren, Peyton W. and Cooper S. Mayhall; his mother Mary A. Mayhall of Princeton; his mother-in-law Lois M. Smith of Princeton; and his brother-in-law Jeffrey E. Smith of Patoka.
