He was predeceased by : his mother Linda Brooks (Ramey); and his wife Sandra L. Carlson. He is survived by : his father Jerry M. Brooks, Sr.; his daughters, Melissa Doyle of Summerville, SC and Kristy Smith of Rensselaer, IN.; his grandchildren, Jayme Doyle, Katlynn Andrews (Billy), Sabrina McNorton, Tyler Smith and Tabitha Smith; and his siblings, Teresa Wilson of Rensselaer, IN and Cecil Brooks of Roselawn, IN.. He is also survived by nine great-grandchildren.

In lieu of flowers, the family kindly requests memorial contributions to the American Heart Association in Jerry's honor.
