He was predeceased by: his parents, Edmund S. Kubiak and Emma R. Kubiak (Smith); and his grandson Joseph Burgess. He is survived by: his wife Mary Jane Hautala; his children, Jane E. Johnson (Mark) of Grand Junction, Susan C. Burgess of Las Vegas, Edward Andrew Kubiak (Angela) of Hillsboro, David J. Kubiak (Jennifer) of Bloomington and Timothy J. Kubiak (Elizabeth) of Winnetka; his grandchildren, Kaela Mohler (Nathan), Jacob Burgess, Gerik, Kelsey, Sam Kubiak, Emma Johnson, Evelyn, Eddie Kubiak, Ruby and Casimir Kubiak; and his sisters, Phyllis Campbell of South Bend and Lois Myers of Edwardsburg. He is also survived by several nieces and nephews.
