He was predeceased by: his parents, Charles T. Polk and Mary Jane Polk (Hattery); and his sisters, Linda Polk, Carol Andrews and Connie "Leah" Polk.
He is survived by: his girlfriend Karen Bricker; his daughters, Tracy Polk, Carla Polk, Deana Polk and Angel Bricker; and his grandchildren, Misty, Mariah and Ashton.
