He is survived by : his wife Jeanette Cook (Pace); his children, Glen "Si" Underwood (Jessica) of Greensburg, Eva Robinson of Salem, Joshua Embry of Texas and Jacob Embry of Texas; and his brothers, Lance Cook of Virginia and Larry Cook of Ohio. He is also survived by 5 grandchildren.
