He was predeceased by: his parents, Thomas F. Sr. and Georgine Hazel (McDonagh); and his siblings, Timothy Hazel and Sharon Kolbe. He is survived by: his wife Rebecca S. Hazel (Vaal) of Valparaiso, IN; his children, Brian T. Hazel (Katherine) of Avon, CT, Scott J. Hazel (Abby) of Noblesville, IN and Amy Hazel-Colley (Mike) of Loveland, CO; his sister Carol Morris; and his grandchildren, Ayla, Tate, Easton, Cassidy, Ashley, Rory, Zoey and Kylee. He is also survived by extended family and friends.
