He was predeceased by: his father William F. L. Forrest. He is survived by: his son Andrew Forrest; his siblings, Billy Forrest (Jana) of Augusta, KS and Kim Carr of Albuquerque, N. M.; his stepmother Joyce Forrest of Scammon, KS; his stepsister Janet Humble of Scammon, KS; and his nieces and nephews, Keely, Jessica, Kyler, Kinsley, Dalen, James, Logan, Dalton and Bryce Forrest. He is also survived by several other cousins and close friends.
In lieu of flowers memorial contributions are suggested to the Countryside Christian Church.
