He was predeceased by: his parents, Ignatius Slattery and Beatrice "Snooky" Slattery (Stegman); his son Patrick IV; and his sisters, Kathleen Slattery and Nancy Slattery.
He is survived by: his wife Rhonda Page; his daughters, Amanda (James Dubin) of Merriam and Crystal (Ryan Person) of Kansas City, MO; his grandchildren, Daniel (Michelle Dubin), Nicole (Ryan Johnston), Sheryl (Ben Dobbins), Anthony Slattery, Tyler (Katie Dubin), Darian Jackson, Samantha Easley, Kalen Jackson and Darius Person; his great grandchildren, Jaleah, Lylah, Ryan Jr., Dahlia, Rawley, Daisy, Carter, Emerson, Reiney, Tucker and Evie; and his siblings, Tim (LaRae Slattery), Susie Campbell (Danny) and Lori Litton (Kyle).
