He was predeceased by : his wife Kathleen Tank; his parents, Armin Tank and Henrietta Carey Tank; his brother Armin Tank Jr.; his sister-in-law Jacqueline Tank; his daughter Kelly Tank; his parents-in-law, Anthony and Joyce LaLoggia; and his brothers-in-law, James LaLoggia and Scott Kuney.
He is survived by : his daughter Sara Tank Ornelas (Rene Ornelas); his grandsons, Esai and Matias Ornelas; his son James Tank (Ha Nguyen Tank); his granddaughter Amanda Taylor-Schommer (David Schommer); his siblings-in-law, Thomas LaLoggia and Julia LaLoggia; his nephews, Anthony Kuney and Steven Tank; and his niece Lisa Severnak (Todd Severnak).
In honor of his passion for music, the family kindly requests donations in Douglas's name to the Wisconsin Conservatory of Music, which "guides and inspires children and adults alike to find their voice, discover their desired instrument, and develop their music-making capabilities.".
