He was predeceased by: his parents, Charles Herman and Thelma Louise Graham Smith. He is survived by: his wife Leshie Wallace Smith; his sons, Jonathan Wallace of Lexington, Kurtis Smith of Westwood, Charles Smith (Shania) of Ashland and Drake Smith of Westwood; his siblings, Bruce Smith of Ashland, Juanita Tague of Ashland and Sue Meyer of Hanging Rock, Ohio; and his grandson A. J. Smith. He is also survived by extended family and friends.
