He was predeceased by: his father John D. Smith. He is survived by: his mother Carolyn Atkins Smith of Ashland; his wife Anita Mulholland Smith; and his sisters, Jonda Lockhart (Rob) of Catlettsburg and Jill Thibodeaux (Eric) of Ashland. He is also survived by several nieces and nephews.
