We are sad to announce that on May 16, 2020, at the age of 89, Hazel Claudine Ward Phelps (Ashland, Kentucky) passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her parents, Nicholas J. Ward and Kathleen Perkins Ward; her husband Walter N. Phelps; and her son Neal Wayne Phelps. She is survived by: her children, Yvonne Green (Charles) of Ashland, KY, Sheila J. Barbour (Jeff) of Richmond, IN and Nicholas J. Phelps of Medina, OH; her daughter-in-law Brenda Phelps of Proctorville, OH.; her grandchildren, Jeffrey, Chuck (Nicole), Walter (Eileen), Michael, Dustin, Eric, Stevie, Chris and Paul; and her great grandchildren, Ashley, Dakota, Griffen, Hayden, Will, Haley, Allisha, Hannah, Lily, Lexi, Matthew and Daniel.
After Covid-19 restrictions are lifted, a Memorial Service will take place at Unity Square.
