He was predeceased by: his parents, Onyx Lewis and Lola Brown Lewis; his siblings, Larry Lewis and Judy Lynn Pullem; and his sister-in-law Cheryl Lewis.
He is survived by: his wife Elizabeth "Libby" Legg Lewis of Ashland, KY; his sons, Matt Lewis (Krystal) of Clinton, Adam Lewis of Ashland, KY, Michael Lewis (Krista) of Berea, KY and John Lewis (Nicki) of Wurtland, KY; his siblings, Garry Lewis of Ashland, KY and Sharon Bocook (Randy) of Ashland; his brother-in-law Timothy Pullem of Ashland, KY; his grandchildren, Courtney, Ella, Ryan, Lily, Onyx and Gus; Renee Payne. He is also survived by nieces, nephews, extended family members and dear friends.
