He was predeceased by: his father Stanley "Bo" Leake. He is survived by: his mother Margaret Avanell Gollihue Leake of Ashland; his wife Terri Leake; his daughter Gaebriell Paulynn Leake of Ashland; his sisters, Judy Lee Hayner (Roger) of Hebron, KY, Denise Michelle Leake of Ashland and Heather Earlayne McCoy (Jay) of Christiana, TN; his siblings' children, Stefanie Justice, Brittanny McCoy, Kaitlynn McCoy and Roger L. Hayner; his great-niece Eliza Justice; and his great-nephew Miles Justice.
