She is survived by : her father Collin Clark; her children, Haley Smith of Barbourville and Jacob Johnson of Lily; and her siblings, Scott Clark of Lexington, Brooke Johnson of Lexington, April Clark of Lexington and Lisa Gambali of Somerset, Kentucky.
