She was predeceased by : her parents, Porter Smith and Effie Marie Johnson Smith; her husband John Cosby; her son Ronald W. Mack; her siblings, Charles Smith, Paul E. Smith, Leona Watson, Jennie Dougherty and Madora Sullivan; and her grandson Ronald Mack Jr..
She is survived by : her children, Vera Bowden (Larry) of Louisville and William "Bruce" Mack (Truddy) of North Carolina; her daughter-in-law Rochelle Mack of Louisville; and her sisters, Margie Linton and Claudia McLean. She is also survived by six grandchildren; eight great grandchildren; and five great-great grandchildren.
