She was predeceased by: her parents, Addison Collins and Cora Hall Collins; her granddaughter Rachel Leigh Fraley; and her siblings, Kenneth "Teddy" Collins, Charles Collins, Marcella Collins Williams, Marlene Collins Johnson and Shawna Johnson.
She is survived by: her husband James Michael Forman; her daughters, Amanda Forman Fraley (Matthew) of Bevinsville, KY, Myra Forman Hall (Josh) of Hi Hat, KY and Megan Forman of Bevinsville, KY; her siblings, Ellis Ray Collins (Melissa), Taylor, Elmer and Barbara Green of Melvin, KY; and her grandchildren, Dakota, Chase, Logan, Kaitlyn, Seth, Hannah, Amber, Hope, Elijah, Nathanael and Rebekah Joy.
