He was predeceased by: his son Keith Allen McIntosh Sr.; his grandson Chance McIntosh; and his siblings, Fonso McIntosh (Brenda) of Ransom, James Larry McIntosh (Diana) of Arkansas and Nettie Goins (Tim) of Matewan, W. V.

He is survived by: his wife Kathie McIntosh; and his children, Lisa Godines (Mario) of Central City, Sabrina Mayes (Jason Blatz) of Ratcliff and Priscilla McIntosh (Raymond Lane) of Browder. He is also survived by 12 grandchildren, seven great-grandchildren and a host of loving family and friends.
