He was predeceased by: his father Herbert Gene Mayfield. He is survived by: his mother Florence Mayfield (Bridges) of Cadiz; his children, Chris Mayfield (Ashton) of Murray, Kentucky and Lauren Mayfield of Louisville, Kentucky; his grandchildren, Bradley Mayfield, Camdon Wheeler and Dax Mayfield; and his siblings, Robert "Bobby" Mayfield of Clarksville, Tennessee and Laura Mayfield Fagersten (Scott) of Huntsville, Ala.
