He is survived by: his wife Cathy Middleton; his children, Kellie Tucker (Lloyd) of Shepherdsville, Michelle Carrier (Jacob) of Mount Washington and Jason Middleton of Clermont; his sisters, Patty and Peggy Middleton; his grandchildren, Ryan Tucker, Ethan Rice and Andrea Tucker; and his nieces, Claudia Hicks and Anna Mae Middleton.
