He is survived by : his wife Sandra Kathleen Cornett Polly of Elkhorn City, KY; his children, Jason Earl Polly of Elkhorn City, KY, Kimberly Rene Frost of St. Cloud, FL, Karen Denise Aschenbrener of Tampa, FL and Laura Lea Jackson of Oviedo, FL; his grandchildren, Leela Polly, Makayla Polly, Morgan Frost, Matthias Frost, Kealan Frost, Bailey Nicole Bates and Cale Allbrittion; his great grandchild Brooks Bates; and his sisters, Mary Evelyn Puckett of Elkhorn City, KY and Ruby Jean Cornett.
