He was predeceased by: his parents, Charles "Pete" and Bertha Anderson Phillips. He is survived by: his children, Tracy Southworth (Anthony) and Michael Phillips (Vicky); his brother David Phillips; and his grandchildren, Andrew Southworth, Aaron Southworth (Raegan), Michael "Mikie" Phillips and Nicole Phillips.

In lieu of flowers, memorial contributions may be made to Hospice Care of the Bluegrass.
