He was predeceased by: his parents, Alford Wade and Agnes Wade (Newton). He is survived by: his wife Elizabeth Ann Wade (Hatfield); his children, Laura E. Johnson (James), Richard T. Stofer IV and Dustin D. Wade (Megan); his grandchildren, Logan Cain, Kori Johnson, Caley Wade and Cameron Wade; his siblings, Patricia Carmichael, Norma Phillips, Janet Ernspiker (Steve), Robert Wade and Kenneth Wade (Cindy); and his brother-in-law James L. Hatfield. He is also survived by nieces, nephews, cousins and friends.
The family requests that contributions in David's memory be made to WHAS Crusade for Children.
