He was predeceased by: his wife Carolyn; and his siblings, Lorraine Coots, Barbara Washburn, Evelyn Bowling, Vickie Lynn Busam and Daniel Keith Lewis.
He is survived by: his children, Charles Lewis (Polly) of Wooton, KY, William Lewis (Melissa) of London, KY, David Keith Lewis (Tonya) of Hyden, KY, Angela Pennington (Rodney) of Stinnett, KY and Misty Coots of Hazard, KY; his siblings, Harry Reid Lewis of Wooton, KY, Joan Lewis of Manchester, KY, Loretta Slone Louisville and Donna Lewis of Somerset, KY; and his close friend Leslie Sams. He is also survived by twelve grandchildren; four- great grandchildren.
