He was predeceased by: his parents, Tilmon McIntosh and Mary Ellen Spicer McIntosh; his daughter Shannon Gail McIntosh; and his siblings, Morton McIntosh, Cecil McIntosh, Phyllis Sewell, Ruth Witt, Cathy McIntosh, Delcie Lewis and Dorothy Roberts.
He is survived by: his friend Treva McIntosh of Jeffersonville, KY.; his sons, Harlan "Eddie" McIntosh of Jeffersonville, KY and Timothy McIntosh of Morehead, KY.; and his sister Edith Miller of Salyersville, KY.. He is also survived by nieces, nephews, cousins and friends.
