She is survived by: her daughters, Michelle Crowe (Duane), Melannie Ladd and Mary-Agnes Hayden (Daniel); her grandchildren, Dylan Crowe, Brett Crowe, Elizabeth Roberts (Bradley), Joseph McElroy, Madison Crowe, Hannah Hayden and Noah Hayden; her great grandchild Claire Roberts; and her goddaughter Merre Lynn Woznicki (Rule).
Visitation will be held on Tuesday, July 1st 2025 from 12:30 PM to 2:00 PM at the Johnson Vaughn Phelps Funeral Home (901 Fairview Ave, Bowling Green, KY 42101). A funeral service will be held on Tuesday, July 1st 2025 at 2:00 PM at the same location. An entombment will be held on Tuesday, July 1st 2025 at the St. Joseph Catholic Cemetery (1209 Fairview Ave, Bowling Green, KY 42103).
In lieu of flowers, donations may be made to the Alzheimer's Association in loving memory of Carol Ann Dales.
