He was predeceased by: his parents, Jay Lewis and Maxine Lewis. He is survived by: his wife Sharon Anderson Lewis; his son Chris Lewis; and his siblings, Bill Lewis (Jeannie) and Sally Osborn (Mark). He is also survived by several nieces and nephews.

Memorial donations in Chuck's memory can be made to Palliative Care of Baptist Health Louisville.
