He was predeceased by: his father Warren Sullivan; and his siblings, Brenda Sullivan and Phillip Sullivan. He is survived by: his mother Shirley Sondergeld Sullivan; his wife Chhary Sar; his son Edward Sullivan; his siblings, Mary Tracy, Debbie Sullivan (Chuck) and Mike Sullivan (Debbie); Theresa Oiler (Mike Carter). He is also survived by two grandchildren; one great grandchild.

In lieu of flowers donations may be made to Hospice of the Bluegrass.
