He was predeceased by : his wife Martha Geneva Sullivan Cook; his siblings, Ruth Wilson, Hertis Wilkerson, Reba Cook, Lorraine Cook and Truman Cook; and his parents, Clarence Cook and Vera Cook.
He is survived by : his son Daniel Eugene Cook of Louisville; and his grandchildren, Laurie Reid, Daniel Cook, Brandon Cook and Amanda Cook. He is also survived by 3 great-grandchildren, and 3 great-great grandchildren.
